Once you know that you qualify, it’s time to complete the application. We make the process simple with a downloadable document that includes the Application, Authorization for Disclosure of Protected Health Information, Physician Information Form and Life Insurance Carrier Release Form. Once completed, you may send these to us by email, fax or mail.

We understand that time is of the essence. To simplify the process for you, our Fifth Season advisor will coordinate with your insurers and doctors to get the medical records we need. Once those are in hand, we quickly review your information and, if we determine that you are eligible, we will provide you with a written, no obligation loan proposal.

Once all loan documents are signed and acknowledgements are in place at the life insurance carrier, the loan will be funded and you will receive your cash.
